Boeing failed to provide adequate training, guidance and oversight to prevent a mid-air cabin panel blowout of a new 737 MAX 9 flight in January 2024 that spun the planemaker into a major crisis, the National Transportation Safety Board said on Tuesday.

After a year and half of investigating, the top U.S. aviation investigator formally determined that Boeing and its chief regulator were to blame for the Alaska Airlines blowout.

The blowout aboard Alaska Airlines flight 1282 in January 2024 occurred minutes after it took off from Portland, Oregon.
